LSMSA Online Registration

Please Read this IMPORTANT information before proceeding with the online registration

The LSMSA Online Registration Process is a two step process.

Step 1 - Please complete the following form, providing all of the required information. (required fields are indicated with **). When completed, click on "Submit"

Step 2 -On the confirmation page that is displayed after clicking on "Submit", please select the appropriate Division, and then click on the "Buy Now" button to submit your payment using our secure online transaction processing through PayPal.

NOTE 1 - There will be a $4.00 administration fee added per registration to the cost of registration in order to cover the costs of offering this online registration service. This administration fee will appear on the PayPal invoice as a shipping charge.

NOTE 2 - Registration requires both the form and a matching payment. If a matching payment is not received, the registration is NOT completed, and the form will not be retained by LSMSA for 2012

NOTE 3 - Registrations for 3 or more children in one family must be completed in person at one of the LSMSA registration days in order to take advantage of the family discounted rate.  The discounted rate is not available for online registration
